                    A person, frequently referred to as a scribe, works as a medical scribe when they accompany a doctor on patient visits and record the doctor's notes. The patient's medical history, vital signs, primary complaints, past medical history, physical examination, and any diagnostic or therapeutic procedures must all be recorded by the scribe. The scribe then arranges the information in a medical record in accordance with the requirements of the doctor's practice.

Responsibilities of a Medical Scribe :
