                     A medical scribe is an allied health paraprofessional who specializes in charting physician-patient encounters in real time, such as during medical examinations. They also locate information and patients for physicians and complete forms needed for patient care.

Roles and responsibilities of a medical scribe :

1. Record patient information into patient’s electronic health record (EHR) accurately and promptly during the patient visit.  

2. Accurately document patient history, physical exam, diagnosis, and treatment plan as dictated by the physician. 
3. Perform administrative tasks such as scheduling appointments and updating patient demographic and insurance information. 
4. Assist the physician with ordering tests and medications. 
5. Transcribe physician orders and place orders into the EHR. 
6. Ensure that all documentation is complete and accurate before filing. 
7. Prepare patient charts for upcoming visits and maintain the medical record. 
8. Assist in obtaining patient's consent for treatment. 
9. Remain up-to-date on the latest developments in the field of medicine. 
10. Maintain patient confidentiality and ensure compliance with HIPAA regulations.
